dtoQuestionDefinition

QuestionDefinition is a single intake question to present to the customer. The questionnaire is a conditional tree flattened into an ordered list: each question_uuid appears exactly once, and a question with a non-empty conditions list is a follow-up that should only be shown once one of its conditions is satisfied.
